What is Palliative Care Support Services?

A serious illness affects more than just the body. It touches all areas of a person's life, as well as lives of that person's family members. Palliative care can address these effects of a person's illness.

Physical problems. Symptoms or side effects include:

  • Pain

  • Trouble sleeping

  • Shortness of breath

  • Loss of appetite, and feeling sick to the stomach

  • Home based Palliative care is for patients who are “seriously or chronically ill” as determined by the physician and surgeon in charge of the care of the patient. Patients under palliative care may choose to pursue aggressive treatment options, while enjoying the benefits of extra layer of symptom management and support from the Palliative Care Support team.

  • “Seriously ill” is defined as chronic illness that may result in death, regardless of the estimated length of the patient’s remaining period of life. This differs from Hospice, as the patient has not entered a stage of life where they may be out of treatment options and/or towards the end of life.

  • Delivering the extra layer of care at home allows early detection and intervention of clinical and social change in condition, that could otherwise lead to health crisis. Emerging evidence shows early and timely detection and intervention may reduce the number of hospital visits or 911 calls.

  • Call Your Elder Care Specialist at 916-767-3459. We will provide you with additional information on the program and help you navigate through the process.

  • Speak to your primary care doctor or to the hospitalist and set up the Palliative Care Consultation.
